North Arlington Pediatric's Medical Home

The staff at NAP is working with families to create a medical home for children who have special health care needs (a chronic condition lasting more than one year). The medical home is a relationship between you and the health care professionals who care for your child. A medical home is an accessible, family-centered, comprehensive, continuous, compassionate, and culturally effective way for NAP to partner with you in the care of your child. As part of the medical home, we have formed a team of practice staff and parents to help us promote and provide quality care for children with special health needs.

Care Coordination

As part of the medical home, NAP will offer care coordination services to your family. If you would like to learn more about these services please feel free to contact our care coordinator Laurie Vlcek at (847)253-3600 or via email.
